I need some advice. My 93 500e starts and runs great when sitting flat in the garage, but if it sits on a hill it has a really a rough start? What could be the issue?

I can't think of anything that would only be affected by an incline.
When you say "rough start" do you mean the engine fires immediately as usual, but then misfires? How long does it misfire before running smooth? Or is it a different scenario?
Runs rough like it is not getting gas on slight incline? Then after you drive it it comes back to life? But when you pull out of the garage where it is flat it runs with no issues?
I'm stumped. Maybe try parking on the opposite incline (point downhill instead of uphill, or vice-versa) and see if that makes any difference.
Wild guessing would be the fuel pressure regulator (FPR) but an incline shouldn't make a difference. If the FPR is old or original, it's a good idea to replace as preventive maintenance anyway.
